The only hospitals are located in the bigger cities and therefore are only reachable with a long walk. Furthermore, the poor can not afford private doctors and conventional pharmaceuticals.

Directly after the collapse of the towers, the First Responders searched the debris for survivors and put their own life and health at risk. It is assumed, that the life expectancy of these people has been lowered by around 20 years due to the toxic dust they inhaled.
